Within 30 days: Our admin team reviews your request and deletes the personal information listed below.

Information We Delete

Your sign-in credentials and saved sign-in data

Your profile photo

Contact details and preferences that are not part of a medical or financial record

Saved addresses and account preferences

Face ID and fingerprint data are never sent to us; they stay on your device. App data stored on your phone is removed when you uninstall the app.

Information We Keep, and Why

Some information must be kept by law even after your account is deleted:

Medical records: Medical history, consent forms, Rx orders, SOAP notes, visit recordings and transcripts that form part of the record, and electronic signatures are kept for at least six (6) years after the last date of service, or longer if state law requires (for example, for minors).

Who the records belong to and who created them: The patient’s identifying details, and the name, credentials and signature of the clinician who documented the care, are kept with those records.

Clinician credentialing records: License and credential documents are kept for the same period as the medical records they relate to.

Financial records: Payment and invoice records are kept as long as required by tax and accounting laws.

Legal records: Information needed to resolve disputes, prevent fraud or meet other legal obligations, including a record of your deletion request.

Kept information remains protected, is accessed only when necessary, and is securely deleted when the retention period ends.

Patients: Deleting your account does not delete your medical records. You can request a copy of your records as described in the HIPAA Notice of Privacy Practices (care.checkmate.health/hipaa-consent-agreement).

Deleting Some Data Without Deleting Your Account

If you want specific information removed, such as your profile photo, without deleting your account, email checkmate@checkmate.health and tell us what you would like removed. We will respond within 30 days, subject to the record-keeping requirements above.
